Supplemental Job Displacement Benefit Voucher Services

RehabWest offers Supplemental Job Displacement Benefit (SJDB) Services For those injured employees unable to return to work and entitled to the SJDB, RehabWest's statewide consultants can assist. The services are outlined below.

$400 Package / Services
Initial Evaluation

• One on one
• Provide injured employee with assessments
  to take home for completion

Assessment

• CAPS (aptitude)
• COPS (interest)
• Self Directed Search/SDS (values)
• In lieu of assessment, may develop resume

Counseling/Training Recommendations

• One on one

Report
$600 Package / Services (in addition to above)

• Expanded Vocational Exploration

• Directed labor market information
• Additional training information
• Vocational objective information
